The American Academy of Pediatrics’ medical home model addresses the many limitations of the United States health care delivery system for children. By using this model, Centennial Pediatrics intends to improve the quality of care, reduce health care costs, foster patient- and family-centered care, and improve the coordination of care for your child. You and your pediatrician will develop a plan of care and make decisions for your child’s health care and related services. By using this medical home model, your family will be better able to navigate the health care delivery systems and supportive services and eventually transition your child’s care to adult health care providers.Values

Care will be provided in a setting where your child can gain access to the practice, where the physician is available to speak directly with families when needed, and where all insurances are accepted.
Family-centered Care
The health and well-being of your child is assured through a respectful family-professional partnership. This is best accomplished in an atmosphere of mutual responsibility between the care provider and the family. We will provide complete information to the family and we will support the family, thereby enabling you to actively participate in your child’s care.
Continuous Care
It is our intention that your child has the same health care provider from infancy to adulthood. We will provide age appropriate health assessments and counseling. We will also be involved in discharge planning if your child is hospitalized or receives care from a specialty provider.
Comprehensive Care
Care will be provided by a well-trained physician who is able to manage and facilitate all aspects of your child’s care. Outpatient and inpatient care services are available at all times. Primary preventive care services are provided, including screenings and counseling, based on American Academy of Pediatrics guidelines.
Coordinated Care
Care is coordinated among multiple providers and ancillary services in the medial home. The process of care coordination includes identifying needs, making assessments, setting priorities, communicating, networking, educating, advocating for resources and monitoring.
Compassionate Care
Our verbal and nonverbal interactions reflect expressions of concern for the well-being of your child. All of our providers make every effort to understand and empathize with your child and your family.
Culturally Effective Care
Your family’s cultural background, including beliefs, rituals, and customs are recognized, valued, respected and incorporated into your child’s plan of care. Language barriers will be addressed and written materials provided to your family in your primary language whenever possible.
